3-on-3 Summer League

I, the parent/guardian of the above-named participant, hereby give my permission for my child to participate in the 3-on-3 Summer League program. I understand that there are inherent risks involved in any athletic activity and I accept full responsibility for any injury that may occur to my child while participating in this program.

I understand that my child must follow all safety rules and regulations established by the program staff. I agree to hold harmless and indemnify Manhattan Christian College, its staff, and volunteers from any and all claims, demands, and causes of action of any kind whatsoever arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in this program.

I understand that in case of emergency, every effort will be made to contact me. If I cannot be reached, I authorize the program staff to seek medical attention for my child if necessary.

I understand that the registration fee is non-refundable unless the program is cancelled by the organizers.
